Outpatient Appointment
Patients must read the following information carefully as we are obliged by the law to provide this information to you. In addition, if you have medical insurance, this section will assist you in understanding how you can use insurance to cover your charges.
Meanwhile, if a family member or a third party will be paying your medical bills, it is advised that they go through this page carefully.
Do note that the patient must pay any charges not covered by a third-party payer or medical insurance.
Consultation Fee
My fee for an initial consultation will not exceed __________ and my fee for any follow-up consultation will not exceed _______.
You may need specific tests following the consultation, such as imaging scans or blood tests. Based on the results of these tests, I will be able to make a precise diagnosis for your condition. If the tests are conducted by a private lab, clinic, or hospital, they will determine the fees for the procedures. The bill for these tests will be charged to your medical insurer or you.
If I conduct the tests, I will provide you with the information about the fees.
If there are any fees, which I will charge in relation to any of the tests I advise that you have, I will let you know what those will be.
